BioFraming is the next evolution of breast enhancement.  

This is a concept developed at Austin Plastic Surgeon, and Dr. Johnny Franco takes the concepts of Preservation Breast Enhancement and applies them to fat transfer. Using your own fat (or processed donor fat like Alloclae) instead of traditional breast implants. 

Novel technology like Viality Fat Processing has shown almost 90% fat retention after a year.  The combination of techniques like BioFraming and technology such as vitality has led to predictable, stable results with your own fat.  

This creates natural, sculpted, customizable breasts that still respect your anatomy, your recovery, and your long-term results.

What Is BioFraming?

In a BioFraming procedure that is based on utilizing and enhancing your natural anatomical structures. It utilizes an anatomical and bio-regenerative approach built around your own structural framework.

  • Your ligaments and support tissues are the framework for your enhancement.
  • Your fat (or processed donor fat like Alloclae) is the bio-regenerative enhancement of the breast. 

Instead of cutting through ligaments, disrupting tissues, or forcing an implant into a space that doesn’t naturally exist, as we have done in the past, we have the merger of new technology, innovative concepts, and anatomical utilization to create BioFraming.  

  1. Preservation Techniques
    • Utilizes the breasts’ natural spaces. 
    • Creates a nesting effect
    • Fat is maximized with precision placement using the ultrasound
    • Customized enhancement
  2. Use hydrodissection to gently expand the natural plane under the breast gland.
    • Just under the breast gland
    • Just above the muscle fascia
  3. This plane already exists in your natural anatomy. Utilizing preservation techniques, we are just opening that natural space to keep all the ligaments and natural tissue intact.  
  4. We preserve:
    • Natural ligaments
    • Soft tissue support
    • The retained ligaments and soft tissue create a nesting effect to hold the fat or Alloclae in position.  

Then, in that preserved, protected space, the fat can be precisely placed for the desired enhancement.  

The result is a breast that we can shape more like an implant if you want that look (more upper-pole fullness, more cleavage, more roundness), but that still feels and looks like your own natural tissue because it is fat.  

How BioFraming Is Different from Traditional Fat Transfer

Traditional breast fat transfer was typically performed by injecting fat into multiple planes or layers of the breast.  This can be deep or superficial, most commonly above the breast gland.  This is a more random placement and sometimes more superficial or more random pockets. It can:

  • Be limited in how much volume you can safely place
  • Sometimes create superficial irregularities, oil cysts, or lumps
  • Be less precise in shaping the upper pole and cleavage

With BioFraming, we’re doing something much more structured and intentionally manner that is being optimized by your own natural tissues.  

  • The natural space that is utilized during preservation breast procedures is the same space that is used for BioFraming, except the enhancement is done with your own natural fat or Alloclae:
    • Under the gland
    • Above the muscle
    • Precision Placement with ultrasound
  • The technique leverages the nesting effect that we use in Preservation Breast procedures.  It is a similar concept, but now the nest is filled with fat or Alloclae instead of an implant.
  • Because the tissue is preserved and the space is defined, BioFraming can:
    • Place more volume than in a typical fat transfer with our palpable fat necrosis or cysts. 
    • Sculpt the shape of the breast much more like we would with an implant, as we have a controlled pocket in which to place the fat or Alloclae.
    • Utilize your own natural tissue to our advantage. 
    • Reduce the risk of superficial irregularities and “hard spots” that can bother patients after some fat transfers.

It’s essentially an injectable breast enhancement that behaves more like a structured augmentation than a random “sprinkling” of fat.

Where Does the Fat Come From?

The fat can be harvested or liposuctioned from any areas from which patients want fat removed.  

We perform liposuction from areas you don’t want the fat, such as:

  • Abdomen
  • Love handles
  • Back
  • Inner thighs
  • Arms

That fat is then:

  1. Precisely harvested, and the areas are sculpted to shape the waist or arms (wherever the fat is liposuctioned from for the enhancement).
  2. Precision Processing: with newer systems, Viality has shown almost 90% fat retention after a year. This precision processing with Viality will help clean, concentrate, and prepare the fat in a way that’s more predictable and reproducible.  
  3. The cannula used to inject the fat or Alloclae is inserted through a tiny 1 mm incision, creating a true injectable breast augmentation.  

From there, under ultrasound guidance, we can place the fat in exactly the right BioFraming position to shape and enhance the breast.

What If I Don’t Have Enough Fat? Enter Alloclae.

Not everyone has enough fat for their breast enhancement.  Ozempic breast has become a reality for many of our patients on their weight loss journey.  The GLP-1 medications have been revolutionary for many of our patients, but many of them have also had breasts that have deflated or lost the volume that they once loved.  That’s where Alloclae, the first filler designed for the body, creates opportunities for our patients in a way they have never had before!  

Alloclae is a processed donor fat product that:

  • Can be used when you don’t have enough of your own fat
  • Is available in larger volumes for body and breast enhancement
  • Still behaves like a fat-based filler

We can use Alloclae in the same BioFraming plane, with the same preservation concepts:

  • Preserve architecture

Where Does the Fat Come From?

Most patients love this part.

We typically remove fat from areas you’d happily slim down, such as:

  • Abdomen
  • Love handles / flanks
  • Back
  • Inner thighs
  • Arms

That fat is then

  1. Gently harvested
  2. Carefully processed using modern systems that help clean and concentrate the fat (this improves predictability and quality)
  3. Injected through tiny incisions (about 1 mm) into the BioFraming plane

Throughout the injection, we can use ultrasound to visualize the tissue and see exactly where the fat is going. This adds another layer of safety and precision to the procedure.

Who Is a Good Candidate for BioFraming?

You might be a good candidate for BioFraming if:

  • You want breast enhancement without typical breast implants
  • You prefer a natural, softer look and feel to your breasts
  • You are having liposuction and want to do some regenerative restoration to your breasts
  • You’ve lost volume after pregnancy, breastfeeding, or weight loss (including GLP-1 medications), and want a natural regenerative restoration to your breasts

What Is Recovery Like After BioFraming?

Recovery will vary based on several factors: 

  • How much liposuction is being performed? This is the biggest variable, as the liposuction is the hardest part of the recovery.  
  • Whether we’re using your own fat or a processed donor fat, such as Alloclae. 
  • Whether we combine BioFraming with any other aesthetic procedures, as this can be combined with a tummy tuck or liposuction 360, it is a great natural breast restoration if you are doing liposuction. 

In general:

  • BioFraming with your own fat:
    • You’ll have recovery both in the donor areas (where liposuction is performed for fat harvest) and in the breasts.
    • Most patients are back to light activities in a few days and more normal routines within 1–2 weeks, depending on the extent of treatment.
  • BioFraming with Alloclae only (office-based):
    • Typically done under local anesthesia
    • Tiny injection sites that heal quickly
    • Most patients go back to normal daily activities the next day after the procedure, which is performed in under an hour.
