One of the questions we often get at Austin Plastic Surgeon is whether Motiva round implants can be used in preservation breast procedures. The short answer is yes round implants can be used as part of the preserve’ breast procedures depending on the individual patient needs and desired outcome from the breast enhancement. The original preservation technique was designed using ergo implants which have a different gel to allow them to slide through the tunnel created during preservation procedures. Using the ergonomic implants was how it was initially performed in Costa Rica and around the world, as that is how it was originally described by Manual Chacon the innovator of the procedure. However, we’ve refined and modified the approach in our practice to allow the use of round or ergo breast implants while still maintaining the benefits of preservation breast surgery. This gives patients more options and flexibility to meet their unique needs! This is especially important

Why We Developed This Technique
The goal was simple, to give patients the aesthetic outcome they desire while maintaining the structural and long-term benefits of preserve’ breast procedures. Many of our patients are on their weight-loss journey or have lost volume after pregnancy in their breast, which can create some unique challanges. Although their weight loss journey has been remarkable in many ways, patients sometimes experience changes to their breasts that present unique challenges. These patients often experience thinning of the breast tissue, which can increase the risk of rippling and breast implant visibility with less cohesive breast implants. This is why we utilize 6th generation Motiva breast implants that are 100% filled in many of our GLP-1 patients that have been on their weight loss journey.
The Ergo and Round Motiva 6th generation breast implants have a slightly different cohesive gel that is used in the breast implant. The round implant has a more cohesive gel that is less likely to ripple or show some of the implant edges in our patients who have limited breast tissue or very thin skin.
By combining round implants with the preservation method at Austin Plastic Surgeon, we can help restore volume that’s been lost over time or during your weight loss journey while also reducing the risk of visible rippling after your breast enhancement surgery.
The Concept Behind Preservation
The preservation approach is all about respecting and maintaining the natural anatomy of the breast. This means maintaining the muscle in its natural position and preserving the internal architecture that supports the breast shape. These ligaments are not cut but separated; when they are pushed aside to create space for the breast implant it creates a nesting effect that leads to a natural internal bra for the implants. By keeping these natural ligaments and structures intact, we aim to provide lasting support and stability for the breast implant’s position. This also leads to a more rapid recovery for our patients. Many of our preserve’ breast procedure patients are busy professionals and have limited downtime to recover from their breast enhancement procedure.
This approach is less about a specific ergonomic or round breast implant and more about preserving the patient’s own tissue framework and natural tissue to enhance the long-term result. The breast implant, whether round or ergonomic, is then used to achieve the patient’s specific aesthetic goals while solving the unique challenges the patient may have for their breast enhancement procedure.
Why Round Implants are a Great Option for Some Patients undergoing Preserve Breast Procedures
We’ve adapted the preservation technique so that we can use modern sixth-generation, highly cohesive, 100% gel-filled round or ergo implants depending on the specific needs of the patient. These implants have been extremely useful to decrease some of the challenges that our patients on their weight loss journey or post pregnancy often face, such as lack of breast volume or thinning skin. These implants maintain a natural appearance while reducing visible rippling after augmentation, particularly in patients who have lost breast volume due to weight loss or pregnancy.
For our patients that have experienced volume loss in the breast and want more shape and cleavage to the breast, these implants allow us at Austin Plastic Surgeon to recreate fullness in that also decreases the risk of rippling after the breast enhancement. By preserving the native ligamentous support and combining it with the round implant, patients can achieve both preservation of natural breast tissue and stability and softness, but also decrease some of the challenges, such as rippling, in our patients that have lost volume to the breast.
Preservation Breast Procedures in Combination with a Breast Lift: The Perfect Combination for Post-Weight-Loss and Post-Pregnancy Patients
Many of our patients who have lost significant weight or breast volume after pregnancy benefit from a breast lift with preservation augmentation to restore the shape and functional volume they lost during their journey. Using this technique, we can lift and reposition the breast tissue. Since we preserve these ligaments, the natural supporting structures, we can place the implant where we want it and restore volume only where it’s needed.
By keeping the supportive ligaments intact, we can maintain the implant in an ideal position and minimize the “bottoming out” that sometimes occurs with traditional methods. This results in an elevated appearance that restores your natural, balanced shape and functional fullness
The Evolution of Preservation Breast Surgery
When preservation breast surgery was first introduced, it was performed almost exclusively with ergonomic implants due to the implant passing through a tunnel in the breast since the ligaments are being pushed aside and not cut so that they maintain their structural support. However the procedure continues to evolve to meet the specific needs of our patients. Round, cohesive gel implants have advanced in design and quality; they’ve become an excellent option for our patients that have been on their weight loss journey. Ozempic breast leads to thin skin and loss of volume in the breast. The round implant because of the highly cohesive gel decreases the risk of rippling and restores some of the functional volume that our patients have lost.
In the coming years, we’ll see new generations of breast implants and bio-technology that will further increase the options patients have to customize their breast enhancement journey. These future innovations will:
- Preserve natural anatomy.
- Respect ligamentous support.
- Customize the implant to the patient’s goals
- And have new options.
The preservation method is a philosophy based on anatomy and technique, not a single device. It’s about using the body’s natural architecture to your advantage rather than disrupting it.
Choosing the Right Implant for You
Every patient’s goals are unique. Whether we use a low, moderate (demi), or high (full) profile round implant or an ergonomic implant truly depends on the individual’s anatomy, tissue thickness, and desired aesthetic outcome, not on the label of a particular technique.
By keeping the natural ligaments, ensuring the muscles stay strong, and using new cohesive round implants, we can provide all the advantages of the preservation method while tailoring the outcome to fit the patient’s body and goals especially in our patients that have deflated breasts and thin skin secondary to weight loss (ozempic breast) or after pregnancy.
The preservation procedure has revolutionized breast surgery for many of our patients, enabling them to enjoy natural, full breasts with long-term stability and minimal risk of implant visibility. It has restored or given them the custom cleavage they desire.
Final Thoughts
Preservation breast surgery has changed how we think about breast augmentation. At Austin Plastic Surgeon, we’ve embraced and evolved this approach to make it compatible with the latest round cohesive 6th generation gel implants. This has given patients the option to use ergo or round implants in a variety of profiles to reach their aesthetic goals and achieve their desired aesthetic outcome. This gives patients the best of both worlds, preservation of natural anatomy and the volume restoration they desire for their breast enhancement.
As technology and techniques continue to evolve, one principle remains the same during the journey:
- using the body’s own anatomy to create a natural, long-lasting result with custom cleavage and functional fullness.
